Start Your Digital Intake Application

Are you interested in Urban League of Metropolitan Seattle programs or in need of direct service assistance? We’re here to help! The first step in accessing our ULMS programs and services is to complete the following Support Services Intake Application below.

We look forward to serving you!

STEP ONE: Every potential client and/or program participant must first complete and submit a ULMS Support Services Intake application. 

STEP TWO: Within 2-5 business days of submitting your intake application, our ULMS Agency Resource Coordinator (ARC) will follow up with you to ensure your intake application has been completed correctly, assess your current needs, and provide information about the programs/direct services you may qualify for.

STEP THREE: Once your needs have been assessed and the programs or services you qualify for are determined, you will be connected with a case manager who will work with you to begin addressing your needs. 

You will need approximately 20 minutes to complete your digital intake application.

After your intake application has been received, program enrollment and/or direct service assistance will typically begin within 7 business days. 

Due to the volume of intake applications received on a daily basis, we cannot guarantee assistance on the same day your intake application is submitted. 

The purpose of our Support Services Intake application is to (1) learn more about your current economic circumstances and (2) collect the important information needed to assess which ULMS programs or direct services will best suit your needs! 

Intake applications only need to be completed once per person. If you have completed an intake application within the last year, you do not need to submit another one for additional assistance. 

Submitting multiple intake applications will not expedite the intake process or lead to quicker assistance.

Once you submit your digital intake application, you can expect our ULMS Agency Resource Coordinator (ARC) to reach out to you within 2-5 business days via the contact information provided to review your intake application and discuss next steps.

If you have not received a response from our Agency Resource Coordinator (ARC) within the designated 2-5 business day timeframe, please feel free to call our main office via (206) 461-3792 or send us a message via  intakes@ulms.org for an update on your intake application status.

PLEASE NOTE: intake applications that are not complete, missing information, or contain false information will take longer to review and may result in a delay in direct services or program participation.
